Full Job Description
This post is fixed term for 12 months due to research funding arrangements.
The Research Care Coordinator plays a central role in balancing direct patient engagement and care coordination with administrative support, data management, and research project delivery across all PCN GP practices. The role will involve working closely with practice teams, community partners, and other PCN staff to support service delivery, research projects, and performance priorities., The role will involve working closely with the practice teams, community partners and other PCN staff to support the delivery on services and performance priorities and will work as part of a wider Primary Care Network research team, to support the safe conducting of research and coordination in accordance with Good Clinical Practice guidelines. Primary Care Networks (PCN) are local neighbourhoods where all health and social care services work together to deliver a better experience for their citizens.Aspire PCNwas founded in June 2024 and increased in size from October 2025. We nowworkwithsix lead practices:Aspley Medical Centre,Bilborough Medical Centre,Broad Oak Medial Practice,Grange Farm Medical Centre,Melbourne Park MedicalCentreandSt Luke's Surgeryand one sister site:Assarts Farm Medical Centre.
We havea population ofapproximately 46,000patients.The PCNemploys fellowSocialPrescribingLinkWorkers, Health and Wellbeing Coaches and a CareCo-ordinatorstosupportwork alongside. These roles will work closely together as the PCNPersonalised CareTeam. The team also comprises of Clinical Pharmacists, a Physician Associate and a Dietician.
Our vision is to build and develop collaborative working acrossour PCN workforce,member practices and stakeholders, to support and strengthen the delivery of Primary Care services and improve health outcomes for the local population we serve.
