Full Job Description
- Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad in-depth theoretical knowledge-base.
- Assessing, diagnosing and treating patients with both minor and complex needs, initiating referrals as appropriate and planning clinical care accordingly.
- Performing safe triage of patients when necessary via telephone or face to face.
- Undertake home visits for the practice and the PCN to provide timely assessment, diagnosis, treatment, and ongoing management for patients identified through the networks practices.
- Undertake care home visits for registered patients of the base practice, providing assessment, diagnosis, treatment, care planning and ongoing clinical management as appropriate.
- Deliver clinical sessions within the PCN Same Day Access Clinic, assessing and managing acute and urgent presentations within scope of practice.
- Prescribing medication appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Providing information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
- Prioritising health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Provide leadership, education and training to other healthcare professionals and staff.

NHS Pension scheme 25 days annual additional bank holidays £50,000 to £60,000 full time hours per annum depending on qualifications and experience. Pro rata for part time hours per annum
