Full Job Description

We are looking for an experienced part-time Physiotherapist to work in GP Practices across Long Lane PCN in Hayes as an MSK First Contact Physio for 25 hours per week on a 12 month fixed term contract. This exciting new role (Band 7) will be joining a growing team that includes Clinical Pharmacists and Social Prescribing Link Workers. Successful candidates will have a named GP Supervisor and access to peer learning developmental opportunities. The PCN covers the following practices:

  • Acorn Medical Centre
  • Heathrow Medical Centre
  • Parkview Surgery
  • Shakespeare Health Centre
  • The Pine Medical Centre
  • Willow Tree Surgery
  • Yeading Court, As a First Contact Physiotherapist, the post holder will work as part of a multidisciplinary team across the practices of a Primary Care Network (PCN), with the PCNs GPs and other clinicians and staff, as well as staff and clinicians from community, acute, social and voluntary sectors. They will work directly with patients to assess and treat conditions, manage long-term conditions as well as promote self-care.
  • The post holder will:
  • Provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients' care (including in relation to undifferentiated conditions). This will involve seeing patients, without prior contact with their GP, in order to establish a rapid and accurate diagnosis and management plan.
  • Progress and request investigations to facilitate diagnosis and choice of treatment regime, understanding the information limitations derived from these and the relative sensitivity and specificity of particular tests diagnostic services such as x-rays and blood test, and interpret and act on results to aid diagnosis and the management plans of patients.
  • Deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions.

    The successful candidate will have experience of initial assessments and creating treatment plans. One of our priority focus areas will be around frailty and prevention of falls. Experience in this area would be an advantage.
