Full Job Description

As a newly qualified/registered Practice Nurse, the post holder will provide patient-facing clinical care, advice and support, initially under close supervision while developing confidence and competence in the role. Working within professional, legal and organisational frameworks, they will assess patient needs, identify risks, provide nursing interventions, and support GPs and the wider primary healthcare team in delivering high-quality care.

The role requires the post holder to respond flexibly to the needs of patients, carers and the service, providing advice and guidance within their scope of practice and in line with practice policies, procedures and professional standards. They will contribute to the delivery of services under the PCN DES and Enhanced Health in Care Homes specification, under the guidance of the Lead PCN Nurse.

The post holder will work collaboratively within a multidisciplinary team and may provide support and guidance to colleagues, including Care Coordinators and Health Coaches. They will also contribute to the ongoing development and expansion of PCN nursing services, supporting service improvement and their own professional development as role requirements evolve.

About us

Working Arrangements

The post holder will have a base at Church View Health Centre but will be required to work flexibly across the PCN footprint, which may include working within any of the other surgeries and across other community settings.

As the role requires the postholder to commute across the Wakefield District applicants should either hold a full driving licence, which is valid in the UK, and hold the relevant car insurance to cover travel for business purposes or can be mobile by using other modes of transport for which reasonable expenses will be payable.

Wakefield Health Alliance South Primary Care Network (PCN) comprises five GP practices located in the South of the district: White Rose surgery, The Grange Medical practice, Park Green surgery, Langthwaite Surgery and College Lane surgery. Collectively we deliver health care services to approximately 65,000 registered patients.

WHA South is a forward thinking and supportive PCN that encourages personal development and growth. We are committed to improving patient care with a particular focus on health inequalities across the Network footprint.

The PCN team currently consists of Nurses, Pharmacists, Technicians, Care Co-ordinators, Physiotherapists, HWCs and MHPs funded under the Additional Roles Scheme. Together we encourage an MDT approach to patient care, integrating the vision of neighbourhood health and delivering the same excellent care and support for both our patients and staff across each of our busy surgeries.

We are looking to recruit an experienced Nurse to join our community team for a fixed term initially with the potential to become permanent.

The WHA CaCHT operates across the PCN footprint, under the guidance and support of the lead ANP and GP, delivering enhanced care to the frail and elderly patients as outlined in our Neighbourhood Health Plan. We have a specific focus on LTC's and more specifically Dementia, and work with local community services to support both patients and families where appropriate offering an holistic approach to care.

Hours of work are flexible between 8am and 6pm Monday to Friday.
