PCN Clinical Pharmacist

Experteer Overview In this role you will provide specialist pharmaceutical support within a collaborative PCN pharmacy team, focusing on medicines optimisation and safe prescribing. You will conduct structured medication reviews, reconcile medicines, and manage polypharmacy for complex patients, working closely with GPs and multidisciplinary colleagues. The post supports progression toward Independent Prescribing and professional autonomy. You will contribute to quality improvement, audits, and prescribing strategies across the PCN, improving patient outcomes at scale. Pay / Benefits

• Undertake structured medication reviews for patients with single or multiple long-term conditions, focusing on complex polypharmacy and frailty. Support deprescribing and falls risk reduction; improve medicines administration in care homes. Reconcile medicines post-discharge and coordinate with patients and community pharmacists to reduce readmission risk. Participate in multidisciplinary PCN meetings and manage repeat prescriptions with appropriate monitoring. Implement changes in response to MHRA alerts, shortages, or local/national guidance. Use Eclipse to identify medication-related risk and mitigate harm; provide independent information to patients. Develop Independent Prescribing qualifications and apply prescribing skills within competence. Lead or contribute to quality improvement, audits, and medicines optimisation projects across the PCN. Tasks Experience as a registered pharmacist Experience undertaking medication reviews and medicines optimisation Experience in a multidisciplinary team environment and supporting safe prescribing Desirable: experience managing complex patients with polypharmacy, GP/Primary Care Network experience, quality improvement or audits, mentoring pharmacy technicians or trainees Experience using clinical systems such as EMIS or Eclipse Independent Prescribing qualification or commitment to obtain it Current GPhC registration and CPD evidence Key requirements
