The post holder will provide clinical services for the Enhanced Care & Frailty Team and be responsible for ensuring consistent, high quality person- centred care is delivered to patients in our care homes., The Advanced Clinical Practitioner (ACP) will provide expert assessment, treatment and management of a caseload, using complex communication skills, judgement, discretion and decision-making. They will monitor and assist with improving standards of care, supervising, teaching and supporting colleagues providing skilled professional leadership. Where appropriate they will act as an expert in their field and lead and/or participate in initiatives to improve standards of care for the client group and assisting to advancing the ACP role., The Advanced Clinical Practitioner is service driven, and the role is structured around the Department of Health Guidance across the four domains of Advanced Practice, those being, Now is an exciting time to join our new Enhanced Care & Frailty Team, as we move at pace to expand our services to meet our patient population needs. The EC&F Team will be providing multi site clinics along with home visits within the community & aligned care homes to support our most vulnerable patients when they need it most.

Dover Town PCN is aligned with three practices within Dover, providing health care to our patient population of almost 40000 people.

Our three practices are:

High Street Surgery 100 High St Dover CT16 1EQ. High Street Surgery also have a branch site in Whitfield (Whitfield Surgery), that dispense to certain patients within the village.

Peter Street Surgery 108 Peter St Dover CT16 1EF. Comprises of three surgeries. Buckland Medical Centre Brookfield Place, Dover and Tara Surgery The Droveway, St Margarets which is a dispensing surgery.

St James Surgery 2 Harold St Dover CT16 1SF. Is one of 32 practices within Invicta Health across East Kent & Sussex.
