Patient and Service Administrator

PMLs Collaborative Care Team (CCT) supports seventeen General Practices located within West Northamptonshire, providing proactive assessment and support to people with longterm and complex conditions and their Carers. We undertake holistic assessment within the home environment, identifying needs and priorities with patients and their carers to improve wellbeing, maintain independence with an overarching aim to prevent unplanned admissions to hospital., To provide efficient, accurate, and proactive administrative support that contributes to the effective operation of services. The role holder is responsible for managing routine and moderately complex administrative activities with minimal supervision, applying established procedures, sound judgement, and attention to detail. The role supports effective communication, robust information management, and consistent service delivery in line with company, team, and NHS standards across clinical and non-clinical areas.   PML is a successful not-for-profit, GP-led organisation providing various NHS community and primary care clinical services to patients across Oxfordshire and Northamptonshire. PML holds GMS contracts, as well as being a GP Federation representing circa 50 GP practices covering approximately 650,000 patients.
