Full Job Description

We are seeking skilled and compassionate individuals to join the NHS Children and Young People's Gender Service (London) in the role of Patient Pathway Co-ordinator. The Patient Pathway Co-ordinator will be responsible for ensuring the efficient management of patient journeys through the service, from initial referral to the conclusion of treatment. The role involves working closely with clinical teams, families, and young people, ensuring that administrative processes are streamlined and that patients receive timely access to the appropriate services. This role is critical in ensuring the service runs smoothly, providing administrative support, tracking progress, and managing appointments., The Children and Young People's Gender Service is delivered by a comprehensive, multidisciplinary team, including paediatricians, mental health practitioners, youth workers, nurses, speech and language therapists, and occupational therapists. The service provides an in-depth assessment of each child or young person's needs, culminating in a personalized care plan developed collaboratively with them and, when appropriate, their families or supporters. A range of care interventions is offered to address mental, physical, emotional, psychological, and social needs. The post holder will play a key role in actively managing the administrative processes within the patient care pathway. They will identify and progress the next steps in line with clinical needs, ensuring a smooth and efficient journey for patients. The successful candidate will be someone with experience working within a multidisciplinary team in a healthcare or social care environment, with a passion for supporting young people. A commitment to working in an inclusive and empathetic manner is essential. You should have excellent communication skills, a proactive approach to problem-solving, and the ability to work efficiently in a high-pressure environment. Candidates must have a good general level of education, with English and Maths to GCSE standard or equivalent.,

  • Experience of working as part of a diverse team
  • Experience of contributing to an inclusive workplace culture, Good general education with English and Maths to GCSE standard or equivalent
  • Educated to diploma level or NVQ 3, City and Guilds certificate level in business administration or equivalent
  • Evidence of some formal further training in short courses, experience of a range of admin practices and with significant experience of hospital administration, Significant proven experience of delivering to high standards in a fast-paced high volume customer care environment.
  • Ability to deal with a range of issues independently and efficiently
  • Proven administrative experience
  • Experience of working with children and young people or demonstrate an understanding of the potential needs of the children and young people, Experience of an administrative role within the NHS
  • Experience of supervising staff, Excellent interpersonal skills with the ability to liaise effectively with a wide range of multi-disciplinary staff
  • Ability to work both autonomously and in a team
  • To be flexible in their approach to the work, according to priorities and changing organisational systems
  • To work on site for the majority of the week
  • Must be able to work from a desk environment for extended periods.
  • Ability to deal with difficult service users and challenging situations
  • Demonstrable experience of data collection, presentation, report writing and handling of data quality issues
  • Ability to work with confidential data and information and deal with sensitive issues discreetly
  • Ability to follow complex protocols and feedback to senior staff as appropriate
  • Organisational and time management skills to meet deadlines
  • Possess a 'can do' attitude, with a flexible approach to work and the ability to cope with changing demands
  • Strong IT skills, particularly with relation to Microsoft Office packages, e.g. Word, Outlook and Excel
  • Excellent interpersonal and communication skills face to face, in writing and on the telephone
  • Ability to plan, organise and prioritise own workload and that of junior staff
  • Ability to prepare reports (written and data) using a variety of software packages and present in an easy-to-read format, Understanding of peer review process and governance processes
  • Knowledge of medical terminology

    Following the recommendations of the Cass Review, NHS England has developed a new clinical model for gender identity services for children and young people, emphasising regional delivery through multidisciplinary care centres.
  • The London regional centre, based in London, is delivered by a partnership of Evelina London Children's Hospital (part of Guy's and St Thomas' NHS Foundation Trust), GreatOrmond Street Hospital, and the South London and Maudsley NHS Foundation Trust. The North West regional centre, based in Manchester and Liverpool, is delivered by a partnership of Alder Hey Children's NHS Foundation Trust and the Royal Manchester Children's Hospital (part of Manchester University NHS Foundation Trust). As of 2024, the South West regional centre, based at University Hospital Bristol and Weston NHS Foundation Trust, also went live, further strengthening the national network of hubs. These centres work together to provide high-quality, timely care to young people across the UK, including addressing the national waiting list. Additional centres are expected to launch in the future as the service continues to evolve and expand its reach.     This is a full-time position, 37.5 hours per week, Monday to Friday., Desirable
