Full Job Description

We are pleased to introduce an exciting new Pathway Coordinator post to support the ongoing development and delivery of My Follow Up Plan. As the service continues to grow across tumour groups, this role will be key in establishing a consistent, sustainable and well-coordinated approach to remote follow-up pathways. The successful candidate will work with Cancer Support Workers, administrative teams and clinical colleagues to ensure pathway resources are used effectively and in the best interests of the service and our patients. The role will support the coordination and development of My Follow Up Plan, helping to reduce variation across pathways and strengthen the way follow-up care is delivered. This is an excellent opportunity for someone who enjoys working across teams, improving processes and supporting service development. The postholder will play an important part in helping to shape a clearer Trust-wide operating model, ensuring that administrative functions and Cancer Support Worker responsibilities are well defined, consistent as My Follow Up Plan expands.,

  • Support the coordination and development of My Follow Up Plan across tumour groups.
  • Work alongside Cancer Support Workers to ensure they are able to focus on patient-facing coordination, self-management support, proactive monitoring and escalation.
  • Help establish consistent processes that clearly separate administrative pathway functions from Cancer Support Worker responsibilities.
  • Support the effective use of pathway resources as the service grows and develops.
  • Contribute to improved pathway oversight, reduced duplication and a more consistent experience for patients and teams.
