Pathway Coordinator

Pathway Coordinators are an integral part of the Outpatient team, looking after patients from the point of referral to DHC through to discharge, ensuring the process is both efficient and effective. This is achieved by accurately processing and actively monitoring the flow of information throughout the patient's pathway. The role would suit someone good at multitasking and problem-solving, is empathetic, and has a passion for helping patients. The Pathways team has 11 staff members and is based in the Outpatients department at Dorking Community Hospital., Pathway Coordinators are responsible for ensuring that the patient journey is both efficient and effective. This involves chasing up reports, results, and other documentation from GPs, hospitals, and third-party providers to ensure they are in place before appointments. When processing reports, the Pathways team will highlight any urgent findings to our clinicians to expedite patient care where required. The ideal candidate is someone who can stay calm and pay close attention to detail and can maintain strict patient confidentiality. The successful applicant will help manage the daily workload to support patient care and keep it as smooth and efficient as possible. We will provide the training necessary to use our in-house IT systems. A good working knowledge of Microsoft Office would be beneficial.
