Full Job Description
Pathway Coordinators are an integral part of the Outpatient team, looking after patients from the point of referral to DHC through to discharge, ensuringthe process is both efficient and effective. This is achieved by accurately processing and actively monitoring the flow of information throughout the patient's pathway.
The role would suit someone who is good at multitasking, problem-solving and has an empathetic nature, and a passion for helping patients., Pathway Coordinators are responsible for ensuring that the patient journey is both efficient and effective. This involves chasing reports, results and other documentation with GPs, hospitals and third-party providers to ensure these are in place prior to appointments. When processing reports, the Pathways team will highlight any urgent findings to our clinicians to expedite patient care where required. The ideal candidate would therefore be someone who can remain calm and maintain a high level of attention to detail. The successful applicant will be able to help manage the workload efficiently and effectively on a daily basis to facilitate the patients care being as smooth and efficient as possible. We will provide the training necessary to use our in-house IT systems, however a good working knowledge of Microsoft Office would be beneficial. You will be required to maintain patient confidentiality at all times., A "can-do" attitude is a vital asset at DHC and we are looking for like-minded individuals to work with us who share the same approach and work ethic.
The Pathways team is currently made up of 11 staff members and based in the Outpatients department at Dorking Community Hospital., This is an excellent opportunity to be a part of our organization, as we are going through a period of growth and looking to recruit a Pathways Coordinator to our professional and friendly team.
