Full Job Description
This role is to provide expertise in the specialist clinical area of Pathway 2 rehabilitation and assessment community bed base, as lead clinician and co-ordinator within CCICP. It will involve high level clinical triage and knowledge alongside strategic planning and project management to ensure all strands of Pathway 2 rehabilitation, across the CCICP footprint, is provided in a responsive, timely and equitable service delivery. The post holder will work in conjunction with key partners across CCICP and MCHT,in the planning, development and delivery of Pathway 2 and act as a central liaison and resource for referrals requiring a timely and pathway appropriate response. The role holder will work collaboratively with Care Community Therapy Team Leads to ensure a consistent rehab therapy led model is embedded across multiple sites. Adopting a Home First approach to ensuring patients can return to their own environment, to continue their rehab journey at the earliest opportunity. The role holder's base will be IPOCH Hub in BIU Leighton Hospital, Crewe however they will be expected to travel to D2A bedded sites across the Central Cheshire Integrated Care Partnership (CCICP) footprint.,
- To manage a highly specialist case load, demonstrating appropriate time management and organisation skills.
- To ensure effective day to day organisation and provision of service to meet service priorities across all Pathway 2 sites.
- To provide clinical leadership and support to the MDT, through case supervision, and the development and monitoring of therapy provision and safe discharge of patients in pathway 2 bedded units within CCICP's remit.
- To actively contribute innovate ideas and lead on the development and implementation of service improvements in collaboration with system colleagues and within relevant Pathway 2 groups.
- To contribute to the development and delivery of service objectives, leading on designated projects, focusing on discharge to assess pathways.
