Parkinsons Nurse Specialist - NWAFT

This secondment offers a development opportunity for a motivated registered nurse to progress into a Band 7 role within the Parkinson's Specialist Nursing Service. The post holder will support the delivery of high-quality, person-centred care for patients living with Parkinson's disease, providing specialist assessment, medication management, education, and ongoing support to patients and their families. Working closely with the multidisciplinary team, the role will contribute to improving patient outcomes, reducing avoidable admissions, and ensuring continuity of care across community and acute settings. The successful applicant will develop advanced clinical skills, autonomous practice, and leadership capability while being fully supported by the existing specialist team., The post holder will support the delivery of specialist nursing care for patients living with Parkinson's disease across the Trust. Key duties include undertaking comprehensive assessments, supporting timely medication management, providing patient and carer education, and offering ongoing monitoring to promote stability and prevent deterioration. The role involves working autonomously within the multidisciplinary team, contributing to personalised care plans, coordinating care across acute and community settings, and supporting early intervention to reduce avoidable hospital admissions.
