Full Job Description
Working as part of a regional team, you'll champion Parkinson's UK in your local community by building and sustaining relationships with a diverse network of health and social care professionals, clinics, and community groups. You'll provide a personalised service that empowers people affected by Parkinson's, their families, and carers, to live fulfilling lives and become their own advocates in health and life wherever possible. You'll manage a complex caseload of multiple clients simultaneously, expertly balancing needs-led support with immediate priorities like crisis intervention and end-to-end safeguarding in partnership with local services. What you'll do
- Provide in-depth, person-centred information, guidance, and emotional and advocacy support to empower clients with health, social care, and welfare issues
- Assist clients to identify needs against benefit criteria, navigate complex processes, and provide practical support via a range of channels, including home visits and community settings
- Respond to enquiries through a range of channels and areas based on service need, managing an active client caseload while keeping accurate, up-to-date online client records in line with practice guidelines
- Recognise and respond to potential safeguarding situations using established procedures
- Answer client enquiries professionally using a jargon-free approach and within established timescales, Interviews for this role will be held online on 24 September 2026. - Background and/or experience of providing health and social care information through a range of channels
- Well-developed communication skills including active listening and questioning
- Experience of supporting and empowering people with problem solving, navigating the health and social care system and participating in their own care
- Demonstrable digital competence, with experience of effective use of a range of tools including online case management systems or similar
- Live in the area specified by the role with the ability to travel and work flexibly without reliance on public transport
