Paramedic or ANP / ACP

Poole Central Primary Care Network are recruiting an experienced Paramedic or ANP / ACP to work in our Acute Home Visiting Team on a fixed term 1 year contract.

The successful candidate will be an experienced Paramedic, Advanced Nurse Practitioner (ANP), or Advanced Clinical Practitioner (ACP) who will provide advanced same-day clinical assessment, diagnosis and management of patients within primary care and community settings. The role will involve conducting visits in patients' own homes, delivering high-quality, patient-centred care whilst working autonomously and as part of the wider multidisciplinary team.

This role is up to 37.5 hours per week, worked Monday to Friday, excluding weekends and Bank Holidays. Working days and hours will be discussed at interview with shortlisted candidates., The post holder will support on the day demands in General Practice by undertaking home visits to facilitate timely intervention for patients with acute health needs in the wider context of the patients health. This will involve a holistic assessment of their medical needs. The role will require the postholder to provide autonomous, advanced clinical assessment, diagnosis, treatment and management of patients requiring urgent or acute care within the community setting. This will require the post holder to work closely with the PCN and Practice Teams, as well as Community and Hospital based services and social care teams all serving the Poole Central population.

Poole Central Primary Care Networkis a group of 5 GP Practices in Poole, Dorset, covering a population of over 65,000. The practices work collaboratively to create joined-up, patient centred, community-based care and support for some of our most vulnerable patients andtheir carers. The successful candidate will work in the Acute Visiting Team alongside a team of experienced Paramedics and a Nurse Practitioner and the wider PCN team.
