Full Job Description
This role offers the opportunity to provide specialist palliative care within a well established team, supporting patients with complex needs and ensuring high quality, patient centred care throughout their journey., You will provide specialist palliative care support to patients with complex needs, carrying out comprehensive assessments, managing symptoms, and developing personalised care plans. You will work closely with the multidisciplinary team to ensure coordinated care, support patients and families throughout their journey, and contribute to education, service development, and continuous improvement within the service.
