Full Job Description
The post holder will work as part of the Palliative Care Respite Team and will be responsible to the Team manager. The Post Holder will assist the District Nursing Team in the delivery of high quality community based nursing and social care to patients who have palliative care needs, in some cases approaching the end of life and who wish to remain at home. The post holder will work collaboratively with other members of the Intermediate Tier Services in supporting the patient and carers and provide personal, social, psychological and spiritual care. He/she regularly works without direct supervision in a patient's home. The post holder will work as part of the Palliative Care Respite Team and will be responsible to theteam manager.
