Palliative Care Community Nurse

Providing specialized, compassionate end-of-life care and symptom management for patients with life-limiting illnesses in their own homes.

• Deliver safe, effective, compassionate personalised care.
• Administration of end-of-life subcutaneous PRN medications (PAR & medications to be obtained via GP.
• Work closely withGPs, district nurses, social services, and family caregivers to ensure seamless support.
• Provide Emotional & Psychological support to patients & families.
• Provide 24/7 rapid response care.
• Request for face-to-face review of unstable/deteriorating patients from Hub CNS, or OOH consultant. The patient is then to be placed on virtual ward.
• Complete assessments/referrals
• Manage caseload.
• Maintain/update Electronic/paper documentation.
• Develop personalised care plans
