Full Job Description

The Band 6 Specialist Palliative Care Clinical Nurse Specialist works as an autonomous practitioner with a caseload of complex patients within a team structure. This is predominantly a community post, covering Basingstoke and surrounding areas, and the acute hospital week end rota.

There is access to clinical expertise and oversight to enable the optimal care of our patients 7 days a week. The delivery of a high quality, effective and compassionate care is essential to our organisation., Main duties of the jobWhat we look for:o Someone who is enthusiastic about working patients and their families with complex palliative care needs in the communityo Someone who is able to provide symptom management and psychological and social support to patients and their families
