Full Job Description
St Lukes Hospice is seeking an ambitious and compassionate clinical leader to become our Pall24 Lead, providing strategic and operational leadership to a new service supporting patients and families across six London boroughs.
Based within the Integrated Care Co-ordination Hub in Kenton, youll work at the heart of a multidisciplinary system alongside paramedics, NHS 111 clinicians and Frailty Consultants, helping to deliver responsive, high-quality specialist palliative and end-of-life care.
This new and innovative service will cover 6 boroughs of northwest London and will be based in the Integrated Care Co-ordination hub within Kenton giving you access to a dedicated multidisciplinary triage centre that directly connects you with paramedics, NHS 111 and Frailty consultants., As Pall24 Lead, you will provide strategic and operational leadership to the Pall24 team, working closely with the Director of Nursing and Clinical Services, Medical Director, and multidisciplinary colleagues to deliver a safe, high-quality, evidence-based service. You will play a pivotal role in developing integrated care pathways, strengthening partnerships across health and social care, and driving continuous improvement to meet the changing needs of our community.
The successful candidate will have the opportunity to make a tangible impact on patient care and staff experience and service performance while developing their own leadership skills with a supportive senior leadership team.
As part of the requirements of the NWL ICB model of care, this service will provide support for patients and families 24 hours a day. Therefore, the post holder may be expected to work shift patterns including night shifts as part of the service development.
