Full Job Description

The number of children supported long term ventilation and with tracheostomies has increased in the last 10 years and this it is this area of the respiratory service that is being developed further with this new nurse specialist post. There are approximately 70 children in the Thames Valley region with long term ventilation requirements and a further 20 with tracheostomies. These children are currently supported by clinical nurse specialists as part of a multi disciplinary team. The role will include assessing patients with consultant staff and providing support to children and their families whilst they are inpatients and once they are at home., · Work with the multidisciplinary team and with families and carers to provide expert clinical care and management for children using the paediatric ventilation service (this includes both invasive and non-invasive ventilation) and children with tracheostomies in Oxfordshire and surrounding centres. In particular this role co-ordinates the delivery of the sleep study service to support children with respiratory conditions., · Work with the multidisciplinary team and with families and carers to provide expert clinical care and management for children using the paediatric ventilation service (this includes both invasive and non-invasive ventilation) and children with tracheostomies in Oxfordshire and surrounding centres. He/she will ensure care is given that recognises not only individual medical needs but also their social, psychological and cultural needs. A critical part of this role is to be the hospital key worker for children who are started on long term ventilation in hospital to facilitate timely and safe discharge home. The respiratory unit uses national database-led protocols to assist in the discharge pathway and the post-holder will be responsible for entering data into the national database, and to ensure that the necessary steps required for discharge are undertaken promptly. · Work as a specialist nurse to develop and maintain a tertiary sleep service for children with suspected abnormalities of breathing during sleep. · Work with the paediatric neuromuscular team to provide expert advice and knowledge for children with neuromuscular diseases who are likely to develop respiratory complications. · Work with the paediatric ENT team to provide expert advice and knowledge for children with tracheostomies. · Provide expert advice, support and education to children, young people and their families. · Promote seamless service of care by working in a variety of settings focusing upon inpatient settings, patient's homes, community services and schools, outpatients and regional hospitals. · Enhance the knowledge and skills of other nurses and other health care professionals within the Children's Directorate, regional hospitals and Primary Health Care team. · Ensure the highest possible standards of care are achieved within the paediatric ventilation and sleep service and to review and audit these on a regular basis. · Be expected to play an active role in continuing to develop safe and effective services, and to assist in designing patient pathways to ensure optimal care in the best environment. Along with the rest of the clinical team, he/she will be responsible for ensuring the national peer review standards for respiratory disorders are met and maintained.

  • Paediatric Nurse with minimum two years experience, Experience in providing care for children with respiratory conditions, You must have appropriate UK professional registration.
