This is an excellent opportunity for personal and professional development within a respected and progressive healthcare organisation., This post will be part of a team who care for children and young people with Diabetes. The post holder will work closely with hospital, community and primary care services. The key purpose of the role is to deliver high-quality targeted family support to families of children diagnosed with diabetes. The post holder's responsibilities will include- supporting families with adjustment to diagnosis, providing parenting support, supporting clinics, and forging working relationships with local schools and services. They will also be responsible for delivering group interventions for parents and young people. The support will be provided in the home, community and hospital settings. The post holder will work collaboratively with the family, diabetes team and any professional or voluntary services supporting the family and will be required to develop their knowledge about diabetes and how it is managed in children, and in keeping with treatment and technology advances. The post holder will act as a role model in the delivery of patient care and will provide support to the team in the effective management of the service. The post holder will provide effective liaison with the multi-disciplinary team and other professional colleagues and agencies involved within the child's care. The post holder will be required to act in a manner consistent with the NMC Code of Professional Conduct carrying out their duties in accordance with Trust policies, guidelines and procedures.,

• Provide needs-led emotional, social and practical support to families of children diagnosed with diabetes.
• To assess the needs of children, young people and their families by collaborating with families to develop goals and care plans.
• Support parents/carers, siblings and other family members with emotional and practical adjustment to diagnosis.
• To take responsibility for own workload and prioritise when necessary.
• To undertake duties as required by the paediatric diabetes service and Line Manager to support the daily running of the service.
• To deal with general departmental enquiries, referring to senior Therapy staff as appropriate.
• To prioritise enquires and give information to patients and the public as appropriate.
• To support with the timely ordering of equipment and supplies as require ensuring patients receive the necessary supplies.
• To be able to work in a variety of environments, including outpatient clinics, inpatient wards and community settings.
• To undertake duties as required by the paediatric diabetes service and Line Manager to support the daily running of the service.
• Proactively safeguard the welfare of all children and actively adhere to safeguarding procedures and policies and attend all relevant training that is required.
• To support service users with understanding diabetes technology.
• To support the running of outpatient clinics when required.
• To ensure that patient related information is kept up-to-date with senior colleagues, as per trust documentation policy and notes recording process.
  
  Registered Nurse Associate qualification.
• Active NMC registration.
• Previous experience working with children and young people in a clinical setting.
• Diabetes knowledge and experience.
• Understanding of safeguarding, child-centred care, and transition to adult services.
• Good IT skills, including use of electronic patient records and digital diabetes platforms., Awareness of currently available diabetes technology.
• Awereness of BPT and NPDA.
• Experience supporting patients using insulin pumps and continuous glucose monitoring (CGM) systems.
• Experience of participating in quality improvement or service development projects., To be able to perform under pressure
• To be able to manage own workload
• To be flexible, with capacity to adapt to new workload demands
• To be able to manage own caseload
• Excellent communication and interpersonal skills with children, young people, and families., Experience in an outpatient, community, or specialist paediatric setting.
• Experience contributing to education programmes for children, young people, and families., Possession of valid UK driver's license and access to a vehicle
