Summary
Tasks/duties:
- Monday - Friday Support client with personal care, preparing breakfast, lunch, prompting medication and light domestic tasks
- Support with accessing social groups twice a week.
Main diagnosis:
Posterior Cortical Atrophy (rare type of early onset dementia)
Communication:
Client can struggle to communicate their words and requires frequent prompts and plenty of time/patience when communicating their needs.
