Are you passionate about supporting people to live independent, fulfilling lives? We are looking for a dedicated and compassionate Outreach Support Worker to join our team in Bexley. As an Outreach Support Worker, you will provide high-quality, person-centred support to individuals with Learning Disabilities, Autism, Mental Health needs, and complex support requirements, helping them achieve their personal goals, develop life skills, and maintain meaningful connections within their community. Where you'll be working: Bromley, Bexley & Newham You will provide outreach support to individuals within their own homes and local community. We are looking for support workers to join our amazing team, and to continue delivering outstanding outcomes., Supporting - Activities of daily Living including social activities, personal care, routines, planning and preparation of meals and budgeting. Communicating - effectively communicate using people's chosen methods Engaging - promoting and motivating people we support to enhance their quality of life Specialising - supporting people who may need positive behavioural support approaches Recording - keeping accurate records &reporting changes in health or circumstances of service users to management
