Full Job Description

The post holder is an experienced clinician (Nurse, Paramedic, or other Health Care Professional) who is accountable for the identification, coordination and management of complex callers and care facilities within their geographic area, working closely with their local and/or regional multidisciplinary teams. The post holder will have a key day-to-day advisory and investigative role as a specialist advisor in Care Homes and complex callers. They will also be responsible for developing and maintaining key relationships with internal and external stakeholders including, acute and primary care providers, community-based teams, NHS 111/OOH providers, mental health, social services and Police. Working alongside the clinical area teams, clinical pathways team, and external agencies to identify unmet needs and appropriate pathways for these patients., The role will cover West Yorkshire ICB Care facilities. An interest in the health of our aging population is essential and a commitment to improving the relationship between our health professionals and care home staff to obtain the most appropriate outcome for the patient. Support and guidance will be provided but the practitioner needs to be comfortable working autonomously and managing their own diary.
