Full Job Description
have good IT and communication skills, and have the ability to work independently as well as part of the team. We have a wide and varied caseload, working with patients with long term, Neurological and Orthopaedic conditions, as well as dementia, and advising on manual handling methods to support their rehabilitation needs and their transition home. We encourage integrated working with our NHS colleagues in Multidisciplinary teams and also our DCC partners in the Social Care sector, third sector to deliver a high-quality service in our locality. The post holder will be responsible for the supervision and development of Band 5 Nurses/Therapist and unregistered Nursing and Therapy Support Workers. We are committed to the continuing professional development of all our team members and offer a supportive environment with regular supervision and encourage on-going external learning as well as providing internal CPD groups, training and peer support. Working Pattern: 37.5 hours per week
