You will need excellent communication and IT skills, strong clinical reasoning, and the ability to build effective relationships with patients, families, colleagues, and partner organisations. Our diverse caseload includes patients with neurological, orthopaedic, long-term and complex conditions, including dementia. You will play a key role in supporting rehabilitation, promoting independence, and facilitating safe transitions of care through assessment, treatment planning, and specialist advice. Collaboration is central to our service. We work closely with NHS multidisciplinary teams, Adult Social Care

and voluntary sector partners to deliver seamless, person-centred care across our local communities. As a senior clinician, you will provide leadership, support, and supervision to Band 5 nurses and therapists, alongside nursing and therapy support workers, helping to develop the skills and confidence of the wider team. We are looking for an autonomous practitioner with relevant postgraduate training and experience, who is motivated to contribute to service development and deliver outstanding patient care.
