Full Job Description

Are you passionate about working with people to make key lifestyle changes to improve their health and wellbeing? We are looking for a highly motivated health professional to join our dynamic Health Improvement and One You Team in Tonbridge and Malling on a 6-month basis. We form part of an innovative West Kent Health Improvement partnership with Sevenoaks, Tunbridge Wells and KCC to deliver the One You Kent health service in Tonbridge and Malling.
The successful candidate will carry out one to one health and wellbeing assessments, supporting people to lose weight, move more, drink less and improve their mental wellbeing. You will also deliver a successful weight management programme.
This work will include both face-to-face and online support meetings with clients. Participation in community and other One You promotional events will form key areas of work.
You will work collaboratively with colleagues within the council, voluntary, community and health sectors to ensure that the customer is at the centre of a holistic approach to their health needs.

The post holder will be highly motivated and able to build good rapport with individuals within the community using excellent communication skills. You will have up-to-date knowledge of relevant health and wellbeing interventions and a passion for developing this knowledge base.
