Full Job Description

We are looking for enthusiastic and highly motivated occupational therapist to join our inpatient team. Crocus Ward (Springfield Hospital) is a 19 bedded unit that offers care and treatment for male and female patients over 75 years of age. The service offers Inpatient care for older people with mental health problems, provides assessment and treatment of older people with a range of diagnoses, who require hospital care due to the severity of need. We provide services for patients cannot be cared for in the community or other settings, due to the intensity and expertise of the care required. Within this setting all aspects of physical health, social care needs, and risks are jointly managed by a multi-disciplinary team, which forms part of a planned and integrated holistic system approach to care. You need to have a genuine passion in working with elderly patients and willing to make a difference in their lives. This role is pivotal in supporting the team in delivering care. You will be expected to deliver high standards of care incorporating the recovery approach to cover all aspects of health needs and risks within a safe environment by working closely with the relevant community services. This will involve giving support to individuals to meet their psychological and physical health needs. You will be co-ordinating and evaluating collaborative patient centred care; getting feedback and improving plans with the multi-disciplinary team (MDT).,

  • To promote social inclusion and physical and mental well-being using evidence based practice. This is underpinned by the principles of hope and recovery, health promotion and relapse prevention.
  • To work with a designated caseload identifying occupational therapy goals as part of the overall care plan, using specialist mental health and occupational therapy assessments, including MOHO tools.
  • To consider both physical and mental health needs as part of the occupational therapy assessment.
  • To plan and implement individual and/or group interventions collaboratively, to enable service users to achieve valued goals and satisfactory occupational performance outcome.
  • To work within agreed clinical pathways and protocols.
  • To monitor and evaluate treatment in order to measure progress and ensure effectiveness of the intervention.
  • To establish effective communication networks with users and carers, team members, OT colleagues.
  • To communicate with people in a manner consistent with their culture, background and preferred ways of communicating.
  • To work as a member of the inter-professional team, contributing to decisions with regard to client care programmes.
  • To provide support and education to clients and carers regarding aspects of occupational therapy e.g. equipment fitting, rehab programmes
  • To work in partnership with ICB, LA, voluntary and other relevant agencies to promote a seamless service.
