Occupational Therapist Integrated Care Rotation

You will play a key role in supporting patients to maximise independence, prevent unnecessary hospital admissions, & facilitate safe discharge & rehabilitation within their own homes & local communities.

Apply today & help shape the future of community rehabilitation & hospital avoidance services., You will work across our Community Rehabilitation Hospital, Community Therapy Team & Hospital at Home Service, providing assessment, treatment & rehabilitation for patients following illness, injury or deconditioning

Working within a multidisciplinary team of physiotherapists, nurses, rehabilitation assistants, medical staff & social care colleagues, you will help patients maximise independence, recovery & safe discharge

You will assess & treat patients in their own homes, recommend equipment, adaptations & interventions, support admission avoidance, early supported discharge & rehabilitation closer to home, & contribute to integrated care planning, risk assessment & recovery for patients who may otherwise require inpatient care

Main Duties

Manage complex caseloads across community & rehab settings.

Undertake holistic assessments & treatment planning.

Work with multidisciplinary teams to achieve patient-centred outcomes.

Support hospital avoidance & timely discharge planning

Provide supervision for junior staff, assistants & students

Participate in service development, quality improvement & clinical governance

Contribute to integrated health & social care pathways

We are looking for an enthusiastic, motivated, & forward-thinking Band 6 Occupational Therapists to join our dynamic rotational service across our Community Rehabilitation Hospital, Community Therapy Team, & Hospital at Home Service.

Our neurological services offer rehabilitation and treatment for conditions including Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.
