Full Job Description

We have an exciting opportunity for an enthusiastic Occupational Therapist to join our established Stroke Early Supported Discharge Service. We work closely with acute stroke services in our area and our neurology colleagues in the community to provide continuity of care across the stroke pathway. Our aim is to promote recovery and well-being, whilst empowering patients to self-manage and maximise their independence using health coaching techniques and meaningful activity. We support safe discharges home and facilitate rehabilitation therapy in patients' places of residence., We take a very holistic approach to rehabilitation, across the domains of self-care, productivity and leisure. You will have the opportunity to utilise your occupational therapy skills to the full. As well as conducting assessments and devising rehabilitation programmes aligned to patients' goals, we provide education, emotional support and carer support.

The immediate team includes occupational therapists, physiotherapists, a speech and language therapist and therapy assistant practitioners.

We are looking for a qualified occupational therapist with post-graduate experience working with stroke patients, who can manage their own caseload, whilst collaborating with the multidisciplinary team and partner organisations.
