Full Job Description

The Clinical Beds team provides holistic assessments to patients who require a period of rehabilitation and assessment prior going home after being discharged from hospital.

We work closely as a multidisciplinary team alongside our partner services such as Adult social care and continuing health care to aid patients and families make long term decisions about their health and social care needs.

This is an excellent opportunity to broaden your skill set in a holistic manner working with occupational therapists and mental health nurses along with supporting junior colleagues such as student nurses, assistant practitioners and rehabilitation support workers.

You will need to demonstrate high standards of evidence based clinical practice, have excellent interpersonal and communication skills and be able to prioritise your workload, manage your time effectively, and work flexibly within the team.

You will carry out profession specific, and holistic, assessments and evaluations as required to promote independence and prevent unnecessary hospital admissions. You will be part of the multi-disciplinary team; planning and developing goal focused rehabilitation/reablement programme for adults with a wide variety of complex physical and mental health needs in community beds.

Alongside the multidisciplinary team the successful candidate with plan and develop goal focus rehabilitation/reablement programmes for adults with a wide variety of complex physical and mental health needs, while the individuals are having a period of assessment and/or rehabilitation with the aim to facilitate discharge into an appropriate setting

The ideal candidate will be an experience occupational therapist with knowledge and experience of community services, you will need to be able to prioritise your workload, have excellent communication skills and be able to build therapeutic relations with our partner services such as adult social care and continuing health care.
