An exciting opportunity has arisen for an enthusiastic and motivated Occupational Therapist to join the Integrated Reablement Team, incorporating the Urgent Community Response (UCR) Service, Pathway 1 Discharge to Assess (D2A) and Reablement Services across Nottingham City.

Working within a multidisciplinary team of Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacy Technicians, Assistant Practitioners, Rehabilitation Support Workers and Administrative Staff, you will support patients to maximise independence through rehabilitation and crisis intervention within their own homes. The service aims to prevent unnecessary hospital admissions and facilitate timely, safe hospital discharges.

As part of the UCR service, you will respond to urgent referrals from emergency and out-of-hours services, providing a rapid two-hour response to crises such as falls, functional decline and acute illness. In-house training will be provided, including falls recovery, assistance from the floor and enhanced clinical assessment skills.

The successful candidate will have a strong understanding of health and social care systems, excellent assessment, clinical reasoning and intervention skills, and experience supporting adults with complex physical, cognitive and mental health needs. You will plan, deliver and evaluate person-centred, goal-focused rehabilitation programmes and work closely with partner organisations to achieve the best outcomes for patients.

Main duties of the job

The successful candidate will demonstrate high standards of evidence-based practice, excellent communication and interpersonal skills, and the ability to prioritise workload, manage time effectively and work flexibly within a multidisciplinary team.

You will undertake profession-specific and holistic assessments to promote independence, prevent unnecessary hospital admissions and support timely discharges. Working across community settings, you will plan, deliver and evaluate goal-focused rehabilitation for adults with complex physical and mental health needs.

The role involves supporting the Community Triage Hub, coordinating hospital discharges and facilitating patient flow. You will use advanced clinical reasoning to triage referrals, liaise with acute and community services, and coordinate safe, effective discharge plans.

You will be professionally accountable for your clinical practice, including risk assessments, capacity assessments, discharge planning and equipment provision. You will provide specialist advice, mentorship and clinical leadership, contribute to service development, quality improvement and clinical governance, and support students and junior staff.

The role requires resilience and the ability to communicate effectively with professionals, patients and carers. You will manage complex situations, use IT systems, maintain accurate records, participate in supervision and continuing professional development, and travel across the City as required.
