The Role:

Reablement Occupational Therapists carry out comprehensive, person-centred assessments with individuals who are experiencing difficulties managing activities of daily living at home, including those recently discharged from hospital following a period of ill health. Reablement provides individuals with the opportunity to work towards outcomes that are meaningful to them, promoting independence and confidence in everyday life.

Working within the Reablement service, you will respond proactively and preventatively to Adult Social Care referrals, using Occupational Therapy expertise, evidence-based interventions and a strengths-based approach. Your role will be to enable adults and older adults to remain living safely in their own homes with the optimum level of independence, supporting them to achieve their desired outcomes.

You will offer professional support and guidance to the team, applying your specialist knowledge and expertise to complex cases. You will receive ongoing professional support from a Senior Occupational Therapist or Occupational Therapy Team Manager. The role requires practice in line with the Care Act 2014, HCPC Codes of Practice and all other relevant legislation and guidance.

To be successful in this role, you will have experience of working with people with a wide range of needs, with a strong focus on promoting independence and enabling positive outcomes. Previous knowledge and experience of moving and handling risk assessments, along with developing and delivering Reablement outcome-focused plans, is desirable.

You will have excellent communication skills and the ability to work effectively both as part of a team and independently, using your own initiative in a fast-paced environment. Strong time management and organisational skills are essential, alongside a solution-focused and flexible approach to practice. The role requires a willingness to travel across the county as required and the ability to manage confidential information with professionalism, tact and discretion.

The post requires you to work a shift pattern of 9.00am-5.00pm and 12.00pm-8.00pm, Monday to Friday, with one Saturday worked every seven weeks. There is also a requirement to work bank holidays on a rota basis., The Fluency Duty is outlined in the Code of Practice on the English language requirements for public sector workers. The Fluency Duty for this role is Required. You must be able to speak and provide advice in accurate spoken English to the public.
