Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Are you a motivated and experienced nurse ready to lead high-quality, patient-centred care in the community? Join our dynamic Community Nursing Service in Coxheath, where you'll play a key role in supporting housebound adults with complex and long-term conditions to remain safe and well at home.
As a Band 6 Team Leader, you will manage a diverse caseload, delivering and coordinating personalised care across a multidisciplinary team aligned to Primary Care Networks. You'll triage and allocate referrals, lead on holistic assessments, and ensure safe, effective care delivery-always focused on preventing hospital admissions and promoting independence.
You'll provide expert clinical care including palliative and end-of-life support, IV therapy, wound care, and bladder and bowel interventions. As a senior clinician, you'll act as a confident decision-maker and problem solver, supporting your team to develop advanced skills and deliver best practice.
We're looking for someone with strong clinical expertise, leadership skills, and a passion for empowering patients through education and self-management.

Additional Working Pattern: 7.5hrs a day Monday to Friday with rotational late and weekend shifts
