Full Job Description
You'll support patients in their own homes and clinic settings, helping them stay safe, comfortable and independent. You'll be part of a supportive nursing team delivering planned care across community and clinic environments. Your work will help patients receive the right care at the right time, while ensuring concerns are shared promptly with a Registered Nurse. The role offers a friendly, flexible working pattern across four days, supporting a healthy work-life balance. What you'll do:
- Implement agreed packages of care for patients in community and clinic settings
- Carry out nursing procedures within your scope of practice
- Observe, record and report changes in a patient's condition to the Registered Nurse
- Support patients with dignity, kindness and respect at all times
- Work flexibly across a four-day working pattern to meet service needs
Experience of supporting patients in a health or care setting, or transferable caring skills - Confidence carrying out clinical tasks within agreed guidelines
- A friendly, approachable manner and good communication skills
- Ability to work flexibly and as part of a wider nursing team
