Nursing Associate | Central and North West London NHS Foundation Trust

We are looking for a skilled and compassionate Nursing Associate to join our Home 1st (Urgent Community Response) team. You will deliver high-quality, person-centred care to patients in their own homes, supporting independence and helping to prevent unnecessary hospital admissions. The post holder must be able to deliver delegated clinical care autonomously in patients' homes, including completing clinical observations, recognising and escalating deterioration, administering medicines within scope of practice, and maintaining accurate electronic clinical records, in order to support safe, timely Urgent Community Response and prevent unnecessary hospital admissions. This is a community-based role requiring strong clinical skills, professionalism and a commitment to CNWL values.

• Deliver high-quality, person-centred care to patients in their own homes as part of theUrgent Community Response (Home 1st) team, supporting early intervention and preventing unnecessary hospital admissions.
• Work under the supervision of a Registered Nurse to assess, plan, deliver and evaluate urgent and short-term care for patients experiencing acute deterioration or crisis at home.
• Carry out a wide range of delegated clinical tasks within scope of practice, including clinical observations (NEWS2), wound care, catheter care and medicines administration.
• Recognise, respond to and escalate signs of deterioration promptly, contributing to rapid decision-making within amultidisciplinary urgent care pathway.
• Communicate effectively with patients, carers and partner services, maintaining accurate electronic clinical records to support safe and coordinated care.
• Uphold NMC standards and CNWL values, contributing to patient safety, service improvement and continuous professional development within a fast-paced community service.

The role supports timely assessment, intervention and short-term care to promote independence, prevent unnecessary hospital admissions and identify deteriorating patients early. Clinical Care
• Deliver high-quality, person-centred care to patients in their own homes, promoting independence, health and wellbeing and helping to prevent unnecessary hospital admissions.
• Undertake a wide range of delegated clinical tasks within scope of practice, including clinical observations (using NEWS2), wound care, pressure ulcer prevention, catheter care (including suprapubic), venepuncture, ECGs and blood glucose monitoring.
• Prepare and administer medicines within the limitations of the Nursing Associate role, in line with Trust and national policies.
Urgent and Community Care
• Recognise, respond to and escalate signs of patient deterioration promptly, seeking advice and guidance from Registered Nurses or other registered professionals as required.
• Contribute to rapid assessment and short-term intervention as part of the Urgent Community Response (Home 1st) service, supporting safe and timely care at home.
Risk Management and Patient Safety
• Assist in the identification, assessment and management of clinical risk, including falls, nutrition, moving and handling and pressure damage, in line with Trust policies.
• Maintain patient privacy, dignity and safety at all times.
• Report incidents, risks and concerns using DATIX and escalate issues appropriately.
Communication and Team Working
• Communicate effectively with patients, carers, Registered Nurses and the wider multidisciplinary team, sharing relevant information to support coordinated care.
• Work collaboratively with partner services including health, social care and voluntary organisations within scope of practice.
Professional Practice
• Practise in accordance with the NMC Code, remaining accountable for own actions and working within defined limits of competence.
• Maintain accurate, timely and professional clinical records using electronic systems in line with Trust policy and NMC standards.
• Take responsibility for own learning, reflective practice and continued professional development, meeting NMC revalidation requirements.
• Support and contribute to the learning and development of students, trainees and support staff as required.
Governance and Service Improvement
• Participate in audit, service improvement and quality activities within the service.
• Use equipment and resources safely and economically, reporting faults, hazards or equipment failure.
• Comply with Trust policies relating to safeguarding, infection prevention and control, confidentiality, information governance, equality and diversity, and health and safety.
