Full Job Description

As a Nursing Associate within the Community Mental Health Team (CMHT), you will play a key role in delivering high-quality, recovery-focused care to adults experiencing a range of mental health difficulties within the community. Working as part of a supportive multidisciplinary team, you will help service users to achieve their goals, promote independence, and support positive mental and physical health outcomes.,

  • Supporting the assessment, planning, delivery and review of care for individuals with mental health needs.
  • Building therapeutic relationships with service users, carers and families to promote engagement and recovery.
  • Monitoring service users' mental and physical health, recognising signs of deterioration and escalating concerns appropriately.
  • Supporting the completion of physical health monitoring and promoting healthy lifestyles.
  • Contributing to risk assessment and risk management plans, ensuring concerns are appropriately identified and acted upon.
