Full Job Description

We are looking for a skilled and compassionate Nursing Associate to join our Home 1st (Urgent Community Response) team. You will deliver high-quality, person-centred care to patients in their own homes, supporting independence and helping to prevent unnecessary hospital admissions. The post holder must be able to deliver delegated clinical care autonomously in patients' homes, including completing clinical observations, recognising and escalating deterioration, administering medicines within scope of practice, and maintaining accurate electronic clinical records, in order to support safe, timely Urgent Community Response and prevent unnecessary hospital admissions. This is a community-based role requiring strong clinical skills, professionalism and a commitment to CNWL values.,

  • Deliver high-quality, person-centred care to patients in their own homes as part of the Urgent Community Response (Home 1st) team, supporting early intervention and preventing unnecessary hospital admissions.
  • Work under the supervision of a Registered Nurse to assess, plan, deliver and evaluate urgent and short-term care for patients experiencing acute deterioration or crisis at home.
  • Carry out a wide range of delegated clinical tasks within scope of practice, including clinical observations (NEWS2), wound care, catheter care and medicines administration.
  • Recognise, respond to and escalate signs of deterioration promptly, contributing to rapid decision-making within a multidisciplinary urgent care pathway.
  • Communicate effectively with patients, carers and partner services, maintaining accurate electronic clinical records to support safe and coordinated care.
  • Uphold NMC standards and CNWL values, contributing to patient safety, service improvement and continuous professional development within a fast-paced community service.
