Do you want to join an innovative, compassionate and forward-thinking community service with a strong commitment to high-quality, person-centred care?We are looking for a motivated MS/MND Community Nurse Specialist to join our South East Staffordshire Community Rehabilitation Team at Midlands Partnership NHS Foundation Trust (MPFT). You will support people across Tamworth, Lichfield and Burntwood from diagnosis through to end of life, based in Lichfield. About the RoleYou will provide specialist, holistic nursing support for people with Motor Neurone Disease (MND) and contribute to the care of individuals with Multiple Sclerosis (MS). You will hold a dedicated MND caseload and support MS activity, working closely with the MS Community Specialist Nurse. Key responsibilities include:o Delivering neurological assessments and personalised care planso Providing symptom management, counselling and education to patients and familieso Coordinating care across the MDT and partner agencieso Supporting MS disease-modifying therapy (DMT) monitoring and patient educationo Contributing to service improvement and clinical audito Supporting end-of-life care planning in line with local protocols This rewarding role offers the chance to provide continuity, specialist knowledge and compassionate care throughout the patient journey.Main duties of the job Why Join Us?     You will be a Registered Nurse (Level 1) with:
  • Significant post-registration experience in neurological care
  • Confidence in managing complex, progressive and degenerative conditions
  • Advanced clinical assessment skills, or willingness to develop these
  • The ability to work both independently and collaboratively within a diverse MDT
  • Excellent communication skills, with the ability to sensitively explain complex information
  • A car driver's licence, due to the community-based nature of the role
