Full Job Description
This is an exciting role for a nurse who enjoys autonomous practice whilst working within a highly skilled multidisciplinary team. You'll support patients in their own homes and community settings, work collaboratively with GPs, district nurses and wider healthcare professionals, and play an important role in shaping the future delivery of community palliative care services., You will be a part of our Community Palliative Care Team (CPCT) as a Community Nurse Specialist. Working at the heart of community palliative care, you'll provide expert clinical support, symptom management and advance care planning for patients and their families, helping them live as well as possible with their condition.
