Nurse Practitioner

Provide a holistic approach to patients, including assessment, diagnosis, planning and implementation of high-quality care. To work collaboratively within the team to meet the needs of patients using evidence based care principals. Liaising with the Nursing & AHP Clinical lead, responsible for ensuring required audits are completed and all practice staff comply with local and national policy., Please note, for Secondment roles before an application is submitted, please ensure you have received the appropriate approval and completed documentation required beforehand, as this may delay the process if an offer was to be made.", The post holder will provide autonomous urgent care to patients who present to the Health Centre. The post holder will:

• Work across NPC Wellway sites, we have surgeries at Morpeth, Pegswood, Lynemouth & Newbiggin
• Be able to work autonomously within an extended nurse practitioner role.
• Provide a holistic approach to individuals including assessment, diagnosis, planning and implementation of high quality care up to the point of discharge or referral to another specialty
• Support to manage the department- HR responsibilities- Contribute to the business planning process
• Supervise and educate junior staff
• To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promoteownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries
  
  Northumbria Primary Care Ltd (NPC) is a pioneering partnership with local GPs and now manages several GP practices with around 120,000 registered patients in Northumberland and North Tyneside.
Whilst appointees will have a base practice within NPC,they may be asked to work at other practices across our group. We do however limit this as much as possible as we appreciate continuity is vital to effective primary care. Please note that NPC is a wholly owned subsidiary of Northumbria Healthcare NHS Foundation Trust.
