Full Job Description

We have an exciting opportunity for a Band 4 Nursing Associate / Assistant Practitioner to join our Community Mental Health Team based at Glanville House, Bridgwater. You will play a key role in supporting patients with severe mental illness through our Depot, Clozapine and Wellbeing Clinics, delivering high quality person centred care within a supportive, established and passionate team. You will work independently under the guidance of Registered Nurses, contributing to holistic mental and physical health assessment, treatment and monitoring. This role is ideal for someone who thrives in a fast paced clinical environment, enjoys collaborative team working, and is passionate about improving the lives of adults with complex mental health needs. As part of our friendly team, you'll also have the option to get involved in team activities such as team days/nights out, sweepstakes, Secret Santa and other wellbeing events - or simply enjoy the warm and inclusive atmosphere day today.,

  • Support the running of Depot, Clozapine and Wellbeing Clinics, delivering safe and effective clinical care.
  • Undertake venepuncture, physical health monitoring, and ordering medication as required.
  • Work closely with pharmacists, Registered Nurses and wider MDT colleagues.
  • Provide high quality, evidence based nursing care within your scope of practice.
  • Monitor and respond to changes in a patient's mental and physical health, escalating concerns appropriately.
  • Maintain accurate and up to date patient records in line with trust policies and NMC standards.
  • Help maintain a safe, clean and well organised clinic environment.
  • Support patients to improve and maintain their mental, physical and emotional wellbeing.
  • Work collaboratively with colleagues to support safe discharge planning and continuity of care.
  • Contribute to audits, quality improvement and clinical governance activities.
