Join our CQC-rated Outstanding Trust as a District Nursing Sister within the Windsor and Maidenhead Community Nursing Team and play a vital role in delivering exceptional care to patients in their own homes. This is an exciting opportunity for an experienced and motivated Nurse to combine clinical expertise with leadership responsibilities. Working under the delegation of the Team Leader, you will manage a clinical caseload while providing day-to-day leadership and management to the District Nursing team. You will ensure high standards of patient care, promote evidence-based practice, and support the delivery of safe, effective, and responsive community services. As part of our supportive Community Nursing Team, you will assess, plan, implement, and evaluate care for patients with a wide range of health needs. You will be passionate about delivering person-centred care, enabling patients to remain independent and receive treatment in the comfort of their own homes. • Maintain required competencies, as identified in the Band 6 Community Nurse competency framework, in order to effectively undertake patient assessment, care planning and immediate care needs.
• Support staff to complete holistic assessment and reassessment of all health and nursing needs, ensuring that every patient has an overview of health and nursing need recorded.
• You will manage a complex and diverse caseload of patients
• Support teams to provide information to patients, their families, and carers both formal and informal, so that they can make informed choices about current and future care needs.
• Provide clinical support, leadership, and guidance to all staff to ensure that evidence-based care and clinical practice are maintained in Community Nursing Teams.
