Full Job Description
The Non-Weight Bearing Pathway is a countywide service supporting patients following a fracture from hospital discharge through to clinical optimisation and progression to weight bearing status. Working as part of the Urgent Community Response Team, the Rehabilitation Technician will provide coordinated support throughout the patient's rehabilitation journey, helping to ensure timely assessments, rehabilitation interventions, and onward referrals where required. The role involves working closely with therapists, fracture clinics, patients, families, and partner services to deliver high-quality, person-centred care. Responsibilities include coordinating patient pathways, progressing exercise programmes within competency, providing equipment and mobility aids, maintaining accurate records, and supporting smooth patient flow through the service. The post holder will contribute to multidisciplinary working, pathway development, and continuous service improvement, while ensuring safe and effective care delivery.,
- Coordinate and support patient flow through the Non-Weight Bearing Pathway, ensuring timely progression through rehabilitation and discharge planning.
- Arrange and coordinate initial patient assessments and follow-up interventions.
- Provide rehabilitation interventions within agreed competencies, including progressing exercise programmes and supporting patients to achieve their rehabilitation goals.
- Assess, order and issue low-level equipment and mobility aids, ensuring safe and appropriate use.
- Maintain oversight of patient caseloads and waiting lists, ensuring records are accurate and up to date.
- Liaise regularly with fracture clinics, major trauma centres and other healthcare professionals to obtain clinical updates and support patient care.
- Communicate effectively with patients, relatives, carers and partner agencies through telephone, virtual platforms, written correspondence and face-to-face contact.
