Non-Prescribing Practice Nurse (Learning Disabilities Focus)

The Non-Prescribing Practice Nurse will deliver high-quality, patient-centred nursing care across both practice and community settings. This is a highly autonomous role requiring the postholder to manage an independent clinical workload, undertake comprehensive assessments, and deliver evidence-based interventions within their scope of practice. A significant component of the role involves providing care to patients in their own homes, care homes, and other community settings. The postholder will work closely with the multidisciplinary team whilst exercising professional judgement and clinical autonomy to support housebound, vulnerable, and complex patients. The role is based within and professionally supported by the Home Visiting Team, working alongside GPs, nurses, paramedics, healthcare assistants, community services, and other healthcare professionals to provide coordinated care closer to home. The postholder will also play a key role in supporting patients with learning disabilities, helping to reduce health inequalities and improve access to healthcare services.

• Full UK driving licence.
• Access to a vehicle insured for business use.
• Ability and willingness to travel independently between practice sites, patient homes, care homes, and community settings.
• Regular home visiting is a core and essential component of this role and forms part of the routine workload.
