Full Job Description
We are seeking two motivated and experienced Neurology Specialist Nurses to join our team based at Dereham Hospital and covering North, Norwich and South Norfolk. This role involves delivering high-quality, person-centred care to individuals living with complex neurological conditions, including Multiple Sclerosis, Motor Neurone Disease, Parkinson's Disease, Huntington's Disease, and Progressive Supranuclear Palsy.
Working across community settings, clinics, and patients' homes, you will provide expert assessment, care planning, and ongoing management to support patients in maintaining independence and quality of life. You will play a key role in coordinating care across multidisciplinary teams and partner organisations, helping to prevent unnecessary hospital admissions and facilitate timely discharge.
As a senior member of the team, you will demonstrate advanced clinical and leadership skills, provide education to healthcare professionals, and support the development and supervision of junior staff. The role requires strong communication skills, the ability to work autonomously, and a commitment to delivering compassionate, evidence-based care.
Main duties of the job
o To provide high quality, personalised, ongoing specialist and patient centred care for people with a confirmed neurological disease to maximise quality of life, within their own homes, care homes, the travelling community and in clinics.
o To be responsible for the direct line management of junior staff within the Central Neurological Specialist Nurse team.
o Neurological conditions to be supported: Multiple Sclerosis, Motor Neurone Disease, Progressive Supranuclear Palsy, Parkinson's Disease and Huntington's Disease.
o To be aware of National Policy/Guidance relating to the care of patients with neurological conditions and implement locally, as appropriate.
o To establish clear links with other providers (secondary care, voluntary sector and Social Services) to ensure co-ordinated interventions.
o To provide education to other members of healthcare teams.
