Full Job Description

This is an exciting post for a experienced neonatal nurse to be the manager for a growing team of experienced nurses and health care assistants who work in the Transitional Care unit and care for babies in the community

The Neonatal Outreach and Transitional Care team will play an essential role in the patient's journey and supporting families.

The post holder will be a full time band 7 supervisory manager covering two hospital sites.

The post holder will work in partnership with the Clinical Leads and the teams on the Neonatal Ward/to ensure that high standards of nursing care are provided at all times., As an experienced Neonatal Nurse, the post holder will manage a team of Neonatal Nurses who care for neonates on the Transitional Care Unit and in the Community who have been discharged from Transitional Care or the Neonatal Intensive Care Unit ensuring effective communication is maintained at all times.

They will play a key role in the assessment of patient care needs and commence treatment plans and be instrumental in navigating reviews of the patient, liaising with the Neonatal areas and ensuring care of babies and appropriate escalation of care when required.

The post holder will act as a role model, promoting quality evidence-based practice, developing the Nursing Team by providing education, best practice guidance and support and ensuring effective management of the patient.
