    • This role will be employed through Oxleas on behalf of the Bexley system to support neighbourhood delivery. The role will lead on the implementation of the priorities as set out in the Bexley Wellbeing Partnership Neighbourhood Plan and support the development of the three Local Care Networks with their delivery plan - with a particular focus on improving outcomes and reducing health inequalities. This role will be a key member of the Neighbourhood Transformation Team, which will support delivering transformational change for the Bexley Wellbeing Partnership - working with the three Local Care Networks and the four Primary Care Networks. This will ensure that the partnership better joins up health and care support, improve people's health and wellbeing and close the inequalities gap. The post holder will play a key role in building collaborative relationships between Local Care and Primary Care Networks and Bexley Care and wider stakeholders within the health and care system and local communities.,
  • The post holder will be a part of the Neighbourhood Transformation Team, led by the Head of Neighbourhood System Transformation working the Local Care Networks and will contribute towards the effective delivery of the partnership's objectives and the Neighbourhood Plan.
  • Enable the development of integrated approaches across health, care, and wider partners by facilitating and promoting innovation and transformation to improve outcomes with Local Care and Primary Care Networks.
  • Working with key stakeholders, VCSE, Social Housing, Public Health, Clinical Leads (Clinical Directors), community and mental health providers.
  • Provide the day-to-day project and programme management support to the LCN and associated programmes.
  • Be the lead for the Health Inequalities working with Clinical Leads for Health Inequalities with Public Health in identifying and developing business cases to address reducing health inequalities.
  • Build and strengthen working relationships with key stakeholders, spanning beyond the core partner, that make up the Bexley Wellbeing Partnership and into our local communities.
  • Ensuring appropriate involvement and engagement with service users, local communities, carers and their families and frontline staff in the development and delivery of service redesign - working with partnership engagement team.
  • Role modelling the collaborative leadership behaviours and the principles as set out in the Bexley Wellbeing Partnership Memorandum of Understanding.
