Full Job Description

The ICP is dedicated to improving the health and wellbeing of residents and is committed to working with all of our diverse communities to ensure everyone's needs are met., The purpose of this post is to tackle health inequalities through a neighbourhood health and wellbeing approach, by leading the design and delivery of transformation projects across health, social care, wellbeing services and the voluntary, community, faith and social enterprise (VCFSE) sector. These projects will support Brent's cross-sector neighbourhood approach, ensuring services are more connected to communities, strengthen wellbeing, and provide earlier support to prevent issues escalating.

You will support multi-disciplinary collaboration across the NHS, local authority and VCFSE partners, ensuring that neighbourhood teams work proactively to address the wider determinants of health and improve population wellbeing. This will include key enablers for neighbourhood-level improvement, such as new ways of working, co-location of teams, workforce development, shared data and local insights, and digital tools.

You will work closely with operational managers, partners and residents/patients to improve quality, user experience and performance against key system metrics, ensuring continuous service development and supporting earlier, preventative interventions across neighbourhoods.

You will support the development of robust processes, spread good practice, and champion learning across organisations to drive improved outcomes and reduced health inequalities.

Brent Integrated Care Partnership has a number of exciting opportunities available, supporting the borough-based partnership to deliver Integrated Neighbourhood Teams in a changing and evolving health and care landscape, as well as running a range of health and care transformation projects across the partnership.

In light of the local ICB restructuring, and the selection of CNWL as the Brent "Integrator" function, the Brent ICP Partners are restructuring their team, leading to a number of vacant positions becoming available.

We have a number of roles available, hosted by CNWL but working as part of a wider partnership of health and care organisations in Brent.
